2015 DRS. ANVAR AND PARI VELJI AWARDS FOR GLOBAL HEALTH EXCELLENCE
The Velji Family Foundation annually celebrates global health education, research and service by honoring individuals and projects that demonstrate committed, creative approaches to serving underserved populations. 


AWARDEES RECEIVE
Recognition at the 2016 CUGH Conference in San Francisco, California, taking place April 9 – 11, 2016, and on the CUGH website. Each will also receive free conference registration, a recognition plaque and USD $750 to support ongoing global health activities.


STUDENT / TRAINEE† AWARDS CATEGORIES

 Velji Emerging Leader in Global Health Innovation*: this award honors a student or trainee who created an innovative project with high potential for impact on the lives of people in low-income settings.

 Velji Global Health Implementation Award: honors a student or trainee who implemented and scaled up** an innovation that is having a sustainable effect on the lives of people in low-income settings.


FACULTY AWARDS CATEGORIES

 Velji Emerging Leader in Global Health Innovation*: this award honors a faculty member who created an innovative project with high potential for impact on the lives of people in low-income settings.

 Velji Global Health Education Award: honors a faculty member who worked collaboratively in a low- income setting to establish a sustainable training program that is improving local educational opportunities.


VELJI AWARDS: CRITERIA AND NOMINATION PROCESS

• The Velji Global Health Awards program is a benefit of CUGH Membership.

• Nominations Format: Nominations are accepted only as a single file (Word Document, 5 pages).

• Nominations Information: Nominations must include the following information in the order provided.

• Nominations Deadline: EXTENDED THROUGH JANUARY 10, 2016, 11:59pm PT

1. AWARD CATEGORY (page 1)

       i. Full Name

          (If student, provide student type and year. Ex. GS1 = Graduate School Student, 1st Year)
      iii. Institutional Affiliation (If nominee is a CUGH member through a CUGH member institution, enter that Institution here.)
      iii. CUGH Membership: State Nominee's membership status (Individual Member, Institutional Member).             
      iv. Contact Information: 1) Email Address, and 2) Phone Number

   b. NOMINATING PARTY INFORMATION (page 1) | (Self-nominations accepted):

The Nominating Party may also act as a reference.

2. NOMINATING PARTY STATEMENT (page 2) | (500 words maximum):
   
a. Explaining why the nominating party feels the nominee is deserving of the award; how the candidate has successfully created an innovative and original project / successfully implemented the project / established a sustainable training program; and how the project or actions positively impact the lives of people in low-income settings.

   b. Addressing the following – with descriptions:
      i. Is the innovation / implementation / training program cost effective and replicable?
      ii. Does the innovation / implementation / training program include partners from other disciplines and volunteers and/or professionals from low-income areas?
      iii. Were ethical guidelines followed?

3. LETTER OF SUPPORT (page 3) | 1 page maximum on letterhead, by a faculty member or colleague, as appropriate. 
Letter must be from an individual other than the nominating party.

4. NOMINEE'S CURRICULUM VITAE (pages 4 & 5) | 2 pages maximum. 
If CV is longer than 2 pages, abbreviate to include relevant information only.





* INNOVATION here refers to a novel concept or tool within the last 5 years.

§ NOMINATION PACKET – FULL SPECS
1. Packets must be emailed as a single word document containing no more than 5 pages. 
2. Pages 1, 2 and 3 must be formatted with 1 inch margins (top bottom, left and right), and use a 12 point font, Times New Roman. 
3. Pages 4 and 5 (the CV pages), must adhere to the same parameters if typed directly into Word. CV pages' margins, font and font size are allowed to vary if the content is pasted as an image (ex. Cut/paste from a pdf file to word).

* (PD) Post-Doctoral Researcher/Scholar
* (UG) Undergraduate Student

* To qualify for the Velji Award, STUDENT / TRAINEE cannot hold a faculty position.
