ADSC Rocky Mountain Chapter Golf Tournament

ADSC Rocky Mountain Chapter’s Second Annual Golf Tournament will be held at 8:00 a.m. on Friday, September 14, 2012 at Fossil Trace Golf Course in Golden, Colorado. Fossil Trace Golf Course is located along the Front Range of the Rocky Mountains and is considered to be one of the finest golf courses in Colorado. At only $150 per player, registration includes breakfast, lunch, beverages, gift pack, range balls, green fees w/ GPS cart, and your chance to win several contests and prizes. Release/Waiver
Photograph Release: By submitting the registration form, I hereby release any photographs that may be incidentally taken of me during these events by Seminar Management to be used for any purpose.

Meeting Cancellations and Refunds
All cancellations must be in writing. Prior to Monday, August 13, 2012, ADSC Show Management will allow a 50% refund less a $100.00 processing fee. No refunds after August 13, 2012, 5:00pm CST.
Substitutions of Attendees: You may substitute attendees with prior written approval from ADSC.

PROFESSIONAL DEVELOPMENT HOURS
 

You may earn up to 15.25 PDHs. This form is for your benefit and should be kept with your personal records. In recognition of the fact that an increasing number of professional engineering licensing bodies require members to pursue professional development on a formal basis, this certificate is accepted by most state professional engineering licensing bodies as documentation of your pursuit of continuing education requirements. There are, however, some states that may not accept the general Certificate.
