2017 Diabetes Symposium
 
Learning Objectives

At the conclusion of the symposium, the participants should be able to:

  • Identify and address manageable risk factors for hospital readmission for patients with diabetes (plenary 1)*
  • Apply Treat to Target algorithm to devise the order of use for newer medications for CVD prevention; and when it is not appropriate to implement these medications (plenary 2)
  • Utilize text messaging and computer generated treatment plans to improve A1C control and overall diabetes care (plenary 3)
  • Identify and treat diabetic patients earlier to achieve A1c target as soon as possible to reduce the incidence of micro and macrovascular disease (plenary 4)*
  • Use information in the DM Playbook to improve A1C control for  patients (breakout A1/B1)*
  • Utilize motivational interviewing strategies to successfully address patient needs and concerns about their DM care (breakout A2/B2)
  • Use OPAP advice when considering treatment intensification for remotely located patients to improve therapeutic inertia (breakout A3/B3)*
  • Apply the information gained in the AMIDC program at Riverside to expand our opportunities in addressing inpatient DM care (breakout A4/B4)

Physicians - The Southern California Permanente Medical Group is accredited by the Institute for Medical Quality/California Medical Association (IMQ/CMA) to provide continuing medical education for physicians.

KP/SCPMG - RRLAA is recognized by the State of California as an Accrediting Agency for Continuing Education for Clinical Laboratory Science (Registration #133).  This program is approved for 6 California Contact Hours.  Program number 01-1710.

Registered Dieticians:
  RD’s and RN’s will be provided a certificate of attendance upon completion and submission of the Program Evaluation,.  It is the RD's responsibility to track and obtain the CEU credits through their personal Professional Development process.
