Target Audience

This activity is intended for hematologists/oncologists, advanced practice providers, nurses, psychologists, social workers, and other healthcare professionals involved in the care of adolescent and young adult (AYA) survivors of cancer.

 

    Learning Objectives

    After completing this activity, the participant should be better able to:

    • Describe the unique survivorship challenges AYA patients face during care transitions and integration with primary care

    • Explain the important role of primary care clinicians in delivering long-term follow-up care for AYA cancer survivors

    • Apply communication and care coordination strategies to strengthen the relationship between oncology and primary care teams

    • Access tools and resources to support AYA cancer survivors, their caregivers, and healthcare professionals
