LEARNING OBJECTIVES

At the end of this activity, learners will be able to:

  • Identify barriers and challenges to good outcomes in diabetes management
  • Discuss with patients what resources and tools are available to help assist with challenges in achieving good diabetes outcomes
  • Learn how the social determinants of health impact the daily lives of our members with diabetes and best practices for addressing them
  • Recognize cultural and other differences in our diabetes patient population that may play a role in engagement to care
