All on-site registration will incur a $25 additional fee. Register early to avoid this charge. All on-site registration fees must be paid by credit card. Cash and checks will not be accepted.

Non-registered guests and children are not permitted in the meeting and conference meal functions.

*PLEASE NOTE: Symposium attendance is open to all health care professionals with the exception of certain employees and healthcare professionals employed by pharmaceutical and medical equipment companies. Representatives from such companies may attend for educational purposes only. No selling, product education, or promotional activity of any kind is allowed at this CME symposium. This includes company name badges or any company branded apparel. Please note that verification may be required.

CANCELLATION/REFUNDS
