LEARNING OBJECTIVES

At the end of this activity, learners will be able to:

  • Coordinate care between hospital and Primary Care Physicians to reduce hospital readmissions
  • Risk stratify patients with Atrial fibrillation (Afib) to improve patient outcomes
  • Treat male patients with various conditions that occur throughout the aging process
  • Utilize updated guidelines in allergy therapy for asthma, stroke care and pharmacotherapy in weight management

ABIM Maintenance of Certification (MOC)

Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 6.5 MOC points in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider’s responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.

Participation information will be shared with ABIM through PARS.

CERTIFICATE OF PARTICIPATION

Non-Physician Participants:  Attendees can report up to 6.5 hours of participation towards their accrediting body (licensure). In lieu of the provider number Attendees can write ‘CME’ in the space provided, if applicable.
