Substitute Session 1: Panel Discussion: Case Studies in Care Transitions: HH, SNF, LTAC, IRF
ABSTRACT
This panel discussion provides HH, SNF, LTAC and IRF transition case scenarios to encourage an interactive guided discussion focused on current challenges within case management practice. Facilitated by a moderator and panelists representing the scope of multisite case management practice, audience members are invited to share best practices and solutions from their practice sites.

1. Identify key challenges/barriers when transitioning to HH, SNF, LTAC or IRF.
2. Describe creative approaches, solutions or best practices you have applied.
3. Through interactive discussion, identify at least one new idea to apply within your setting.

By utilizing a solution-searching approach, this session is designed to provide attendees with key strategies to approach difficult problems/challenges. You can expect to learn tactical and sequential steps to the process of A3 solutions through the Plan, Do, Check, Act approach.
