Program Details

As active treatment comes to an end, many patients and families struggle with the transition from regular visits with the oncologist to long-term follow-up with a primary care provider. Join us for a conversation on the relationship between the oncologist and primary care provider and how everyone can better work together. Our panelists will address:

  • Survivorship care plans
  • Keeping open communication
  • Strategies for staying healthy after treatment

Participants will have the opportunity to ask questions and chat with our speakers virtually.
