Please join us for an informational session on how the SCCOE can support schools in optimizing student well being and success upon returning to school after an extended absence due to physical illness, mental illness, or a hospital stay.
We will provide an overview of levels of support and services offered, from consultation and collaboration to full SCCOE coordination of care.
The intended outcome of our work is to ensure students experience a positive transition back to school and to facilitate productive communication and effective collaboration between families, schools, and medical providers.
