If you are unable to attend the Golf Tournament, we would welcome your donation to the Scholarship Fund. Tax receipts are available. Please call the Ontario Long Term Care Association at (647) 256 3490 to make your donation. Thank you!
Commercial Members are eligible to register up to one foursome and must be a sponsor of the tournament. This allows regular members to have an opportunity to play.

CANCELLATION POLICY:
Cancellations must be received in writing (kirsten@bayleygroup.com) by August 8th for a refund less $50 administration fee. NO REFUNDS after this date unless Doctor's note provided. Substitutions are permitted at no additional cost.
