- Wednesday Opening KEYNOTE: Dr. Dave Williams
Physician & NASA Astronaut
A true Canadian hero, physician and astronaut, Dr. Dave Williams has a unique point of view on our conference theme, Leading the Shift: the Power of Thinking Differently.
 
Dave is an ER doctor, a former astronaut and Director of Life Sciences for NASA, and currently the CEO of Southlake Regional Health Centre. In 1998, he became the seventh Canadian in space and in 2007 set a Canadian record for most spacewalks during a mission aboard the International Space Station. (Considering how much time he's spent in space, he's surprisingly down-to-earth!) Given his leadership in these dynamic and high pressure roles, Dave has plenty of firsthand experience with change and risk management. He understands how to build consensus and respect between those you work with and those who work for you. Whether constructing a space station or strengthening the health care system, Dave knows it takes a team of people working together creatively, with common purpose, to truly make a difference.

 
- Thursday Opening KEYNOTE: Dr. Danielle Martin
Vice President, Medical Affairs and Health System Solutions
Women's College Hospital
Danielle Martin is a family physician and Vice-President, Medical Affairs and Health System Solutions at Women's College Hospital (WCH). She is also an Assistant Professor in the Department of Family and Community Medicine and the Institute for Health Policy, Management and Evaluation at the University of Toronto. 

Danielle is a national leader on issues relating to health care policy, working as an expert both within and outside academia. She holds a Masters of Public Policy and is an expert on issues relating to the preservation and enhancement of public health care as well as the intersection of primary care with other parts of our health and social systems. She is the medical correspondent for CBC’s The National, a columnist for Chatelaine magazine and an in-demand speaker on the future of Canada’s health care system. 
In 2006 Danielle helped launch Canadian Doctors for Medicare (CDM) and chaired the board of CDM until May 2013. She sat two terms on the Health Council of Canada and is a recipient of the Canadian Medical Association Award for Young Leaders. In 2013 she was named by the Toronto Star in its list of the "13 People to Watch.” 

In her work at WCH Danielle is helping to advance the vision for Women’s College Hospital as a “hospital designed to keep people out of hospital”, developing and evaluating new models of care that deliver solutions to the most pressing issues facing Canada’s health system.

Susan Anstice

Susan Anstice is Manager of Coordinated Care Planning for the Don Valley Greenwood Health Link at WoodGreen. Susan has 20 years experience in community services, including research in the area of addictions and mental health. More recently Susan was a Clinical Social Worker supporting older adults in East Toronto, their caregivers and families, through assessment, counseling, service navigation, advocacy and case management.

 
Breanne Armstrong
Communications and Research Coordinator

Breanne Armstrong joined the team at OCSA in May, as their Communications and Research Coordinator. She holds a Master’s degree in Media Studies from the University of Western Ontario, where her research was focused on online communities, and has worked with a diversity of not-for- profit organizations. She has been developing social media strategy since way back in the days of MySpace.

 
Raquel Betini

Raquel has PhD in Science from University of Sao Paulo, Brazil, specialized in ruminant nutrition. She is currently completing her second PhD in the Aging, Health and Well-being Program at the University of Waterloo, where she is pursuing her passion for better understanding and addressing the needs of caregivers.

 
Paula Blackstien-Hirsch
With over 30 years of experience in healthcare, Paula Blackstien-Hirsch has led system level improvement initiatives provincially and nationally in the hospital and community-based sectors. Paula was formerly Executive Director for the Centre for Healthcare Quality Improvement, Senior Director with the Canadian Patient Safety Institute, Manager of applied research projects at the Institute for Clinical Evaluative Sciences, and is on Faculty at the University of Toronto, teaching in several programs focused on Quality Improvement.

 
Patricia Blakeley

Patricia Blakeley is the Director of Community Support Services at Acclaim Health. Patricia has worked in a variety of roles in social services including program coordination, public education and volunteer management. Having worked with a number of different client groups, she recognizes that excellent client care comes as a result of listening to and respecting a client's wishes and providing support to meet individual needs and goals. Volunteer programs naturally fit with a client first focus and she is a strong advocate of the vital role volunteers can play in supporting the health of their clients.

 
Dr. Kerry Bowman
Clinical Ethicist
Mount Sinai Hospital & University of Toronto.
Dr. Kerry Bowman has a Ph.D. in Bioethics, a fellowship in Cultural Psychiatry and a Master’s degree in Social Work. Dr. Bowman is the Clinical Ethicist at Mount Sinai Hospital and teaches at the University of Toronto.

Dr. Bowman lectures and publishes widely on ethics in health care, community health care ethics, reproductive ethics, end-of-life care, cultural diversity, genomics and emerging medical technologies. He has consulted extensively on these topics in Germany, Iran, South Africa and The People's Republic of China.

Dr. Bowman is also an environmentalist and founder of The Canadian Ape Alliance, is on the board of directors of The Jane Goodall Institute and is a consultant to The United Nations Environment Program. He has done extensive environmental work internationally with particular focus on the interface of human and environmental needs. He is actively involved in conservation projects in North Korea, Central Africa and Brazil.

 
Catherine Brookman

Catherine is a consultant with 30 years’ experience in health care organizational management, partnership exploration, program development/evaluation, research, knowledge dissemination and translation. Catherine is a dynamic authority on Ontario’s Personal Support Worker (PSW) occupation with extensive expertise in gerontology specializing in the development and evaluation of services for seniors. 

Catherine is working with University of Waterloo CRE-MSD, the Aphasia Institute, St. Clair West Services for Seniors and McMaster University as a co-investigator for the Ministry of Labour funded PSW health and safety research project. She holds a Doctorate, Masters and Bachelor of Science degrees.

 
Rodney Burns , CPHIMS-CA

Rodney Burns received a B.Sc. (Hons) in Medical Physiology from the University of Western Ontario in 1988 and a Masters of Health Administration from the University of Ottawa in 1992. For the past 20 years, Rod has held senior leadership positions in Information Management portfolios developing and implementing multiple IS and corporate strategic plans. Rod joined the Association of Ontario Health Centres in 2011 to lead a unified information management strategy for Community Health Centres and other community-governed primary care organizations across the province, including the implementation of Canada’s largest electronic medical record projects. Rod joined the Ontario Community Support Association to provide Chief Information and Privacy Officer leadership in 2014. Rod is a Certified Health Executive of the Canadian College of Health Leaders, and holds a senior designation with the Health Information and Management Systems Society. He was also one of the first Canadian professionals to earn the CPHIMS-CA designation.

Dan Carbin

Dan Carbin has more than 15 years of private sector advisory and public sector policy experience. Dan co-founded Santis in early 2012 after more than seven years in the Ontario government, including serving as a senior advisor to the Premier, Deputy Premier and two Ministers of Health. Dan helped drive forward some of the government’s signature initiatives including the Wait Time Strategy, the creation of Local Health Integration Networks (LHINs), primary care reform, and physician services negotiations. 
At Santis, Dan provides strategic advice to Boards and executives including health care providers, leading national and provincial health care associations, broader public sector organizations, Fortune 500 companies, and charities. Among his community sector projects, Dan co-lead the development of the OCSA’s 2016-2019 Strategic Plan (to be released this fall) and wrote a White Paper to government on behalf of OCSA on the association’s position on the Bringing Care Home recommendations. 
Dan currently serves as Chair of the Board at St. Jude Community Homes. He has also guest lectured at University of Toronto’s Dalla Lana School of Public Health. Dan has an MSc with Distinction from the London School of Economics and a BA Hons from McGill University.

Lori Cooper

Lori has been with the Victorian Order of Nurses for eighteen years, for the past six as the District Executive Director of VON Community Support Services for South Eastern Ontario. Lori’s previous roles within VON over the past eighteen years included managing regional community support services, an interim assignment as VON National Director of Volunteer Services and leading teams through several structural and process changes. 
Lori has been a champion of the implementation of the interRAI Community Health Assessment, first in partnership with agencies in the SE LHIN, when they were early adopters and then serving on the provincial CSS CAP Implementation Steering Committee. 
 Models of care that support client directed care have been a focus for Lori over the last several years. The VON Seniors Managing Independent Living Easily (SMILE) Program in the SE LHIN region is an example of a self directed model. SMILE supports Seniors’ to identify supports and services that will best meet their current needs and how those services will be accessed, utilizing existing home and community care services and /or reaching into their local communities and circles of care for non-traditional helpers. Clients have the ability to direct the budgets provided, to access care. 
Lori lives in Brighton, Ontario and enjoys being active in the community including sitting on the Board of the Belleville Quinte West Community Health Centre, currently serving a term as Board President.
 
Sarah Cormier

Sarah is the Data Management Coordinator with the Champlain Community Support Network. In previous roles with Eastern Ottawa Resource Centre and Alzheimer Society of Ottawa & Renfrew County, Sarah coordinated and delivered various CSS services to clients and their families. She developed a particular interest in how this work is captured, reported and used by funders and agencies. In her time with the Alzheimer Society, she worked as Database Administrator and Statistics Lead and developed a rich understanding and interest in OHRS, MIS, data quality and management. Sarah holds a BA-Honours from the University of Guelph.

 
Jennifer Croft

Jennifer Croft is a Manager of Community Programs for ONE CARE Home and Community Support Services. In her 20 year career she has worked in most sectors of the health care system, including hospitals, long term care, public health and community care. Her favourite aspect of her work is collaborating with partners to support a holistic approach to health services. She is highly dedicated to promoting quality of life and independence for seniors and people with disabilities. Jennifer holds a Masters degree in Health Studies from Athabasca University.

Mary Eastwood is the Director, Health Link Initiatives for the Don Valley Greenwood Health Link (DVGHL), the Health Link led by WoodGreen Community Services. She brings over 20 years of project manager and management consulting experience working in both the public and private sectors including health care. She has experience supporting organizations in the process of successfully carrying out transformational change; this work has involved, for example, project management, work flow /process redesign, and staff engagement. Her work for the DVGHL includes coordinated care planning, community and physician engagement, and supporting quality improvement and change management.

 
Leslie Eckel

For the past 8 years Leslie has worked as a Knowledge Exchange Associate for the Ontario Home Care Chair where she worked closely with home and community care organizations in promoting the adoption of research evidence into practice and policy. She has over 20 years’ experience working with the older adult population in a variety of health care settings. She has a MSW, post graduate studies in KT, and is a PhD (c) in the SPHHS at UW.

Shoshana has 20 years of experience as a gerontologist, clinician and psychotherapist in community settings, long-term care and private practice. This year, she recieved the Baycrest Excellence in Sharing Learning award with the LRI team, for positively impacting staff through experiential learning. Shoshana’s past roles include non-profit management, program coordination and instructional design, content & marketing for e-learning solutions. Shoshana has an MSW (1997) and Diploma in Gerontology (2001) from U of T, and a Specialized Honours BA in developmental psychology from York U (1995).

 
Janine Hopkins

During her 20 years in health care, Janine has held leadership roles in public, not-for-profit and private organizations specializing in system strategy, public affairs and policy. 
From 2008 to 2013, Janine was Senior Director, Community Engagement and Corporate Affairs at the Toronto Central (TC) LHIN. Janine was responsible for strategic planning, communications, community engagement, and key corporate initiatives including programs to improve care for Aboriginal people and Francophones. 
Janine was Vice President of Public Affairs for Cancer Care Ontario (CCO) from 2005 to 2008 where she played a lead role in developing several Ontario Cancer Plans. As Director of Communications for Ontario’s Minister of Health and Long-Term Care, Janine oversaw the communications and stakeholder engagement strategies for health system priorities including Family Health Teams and the Wait Time Strategy.
Janine has played leadership roles in health system planning initiatives at the provincial, regional and local levels including, leading planning for several community hubs in Ontario and serving as a senior advisor for Alberta Health’s review of the province’s acute care, continuing care and wellness programs. Recently, Janine and Dan supported the Ontario Community Support Association with its strategic plan. While at the TC LHIN, Janine was responsible for delivering the Integrated Health Services Plan and was on the executive committee for the TC LHIN’s Health Links and primary care strategies. 
 Janine was Board Chair of Interval House and a mentor for the Rainbow Health Public Policy Institute. She holds an MA in Political Science (public policy stream) from the University of Toronto.

Tai works at the intersection of healthcare, design and policy. In addition to leading the creative team at OpenLab, Tai is also campaign manager of Choosing Wisely Canada, a national campaign to reduce unnecessary healthcare. He was previously the Director of the Excellent Care for All Strategy at the Ontario Ministry of Health and Long-Term Care where he led the ministry’s quality agenda and oversaw the development of ECFAA.

 
Gurjot Jassy

Gurjot Jassy, BSc, MBA, CHE, works as the Manager of Corporate Planning and Quality Initiatives at St. Joseph’s Home Care. She is responsible for leading projects that will enhance health care services rendered to home care clients. She does so with the support of the Quality & Client Safety Committee members, of which there are client and family representatives to capture the client experience firsthand. She received her undergraduate from the University of Ottawa, and completed her MBA with a Health Services Management from McMaster University. In her spare time, she participates in the Halton Industry Education Council’s Women as Career Coaches event.

Leigh Judson has been working in the CSS sector for 13 years and is the Manager of Supportive Housing for St. Clair West Services for Seniors. Leigh successfully launched 5 Supportive Housing Programs in in Toronto’s West end, delivering Personal Support Staff, Case Workers, Mental Health Consultants and Nursing to clients. In 2010, Leigh identified a gap in the continuity of care for Supportive Housing clients which was addressed through an initiative with Paramedics Services. This innovative program is currently being adopted across TC LHIN.

Zayna Khayat

Zayna Khayat is a senior advisor in health system innovation at MaRS Discovery District, an innovation hub in Toronto with a major focus on the health sector. At MaRS, Zayna plays a critical role in helping get health technology innovations adopted by healthcare systems in Ontario, Canada and around the world. Zayna is also an adjunct professor in the Health Sector Strategy stream at the University of Toronto’s Rotman School of Management. Zayna was previously director of development at the Ivey International Centre for Health Innovation. Prior to this, she had an 11-year career in strategy consulting in the global health and life sciences sector, first with the Boston Consulting Group’s Toronto office from 2001 to 2010, and then as an associate principal with SECOR Consulting. She holds a PhD in biochemistry from the University of Toronto and the Hospital for Sick Children, with a focus on insulin action and diabetes. She lives in Toronto with her husband and three children.
 
John Klich

John has been an Advanced Care Paramedic for over 20 years, working both in the rural and urban settings as well as in the air ambulance sector. John was the first Community Paramedic with Toronto Paramedic Services; in this role he was able to combine his academics (BA Social Science) with his desire to help patients "beyond the 911 call". He now works to integrate paramedic services into the greater health care system and find innovative ways to improve patient experiences.

Jerry is a public and social systems innovator addicted to future possibilities and uncertainties. He is working with families, innovators and experts to create better work opportunities and better community living for today and for the future. He is also working with governments, businesses and communities to make government more open and innovative. Jerry led various foresight initiatives in the Ontario Public Service and the broader public service, from strategic planning to tools development.
 
Dianne Kuipers

A firm believer in teamwork with a strong commitment to community development, Dianne Kuipers is the Capacity Developer for the Community Support Coalition – Eastern Counties & Area. She supports Health Service Providers of CSS in needs analysis, service development, resource deployment, planning, project management, coordination, evaluation, partnership development, knowledge transfer and reporting. Her previous experience in consulting, negotiations and advocacy enabled her to work with a variety of providers beyond CSS including Hospitals, Community Health Centres, the Champlain CCAC and the CMHA. As she has done since 1995, she continues to teach at the Cornwall Campus of Saint Lawrence College.

 
Bruce Libman

Bruce Libman was born in Montreal and studied biology at McGill University and epidemiology at the University of Ottawa. He has extensive background in biostatistics, geographical information systems and experimental design. At the Champlain Local Health Integration Network, he brings his expertise in database queries and data analyses to inform decision makers with the best evidence available from administrative and census data. His interests are in how seniors use the health care system and how to most equitability distribute services to this population.

 
Tracy Lindsay

Tracy Lindsay, MHA (Health Admin) is the Director Strategy, Quality & Outcomes with the Mississauga Halton CCAC. She has held this post for the past three years before joining the CCAC she worked as a health care management consultant for RWS Advisory and nD Insight. She has 18 years of experience in health care management and operations in both the acute and community care sectors, with a particular focus on strategic planning, performance measurement and evaluation and quality improvement.

 
Dana MacMillan

Dana MacMillan is the Director of Services at Wesway. She welcomes opportunities for discourse and to exchange best service practices in respite care. She has a diverse background in child development, sociology and gerontology as well as 26 years of experience in the field of respite care. Dana’s positions at Wesway have focused on creating responsive services for families caring for individuals with challenging care needs from birth to senior years who live in either the city or a rural or remote area. Whether involved in the re-shaping of services, delivery evaluations or system re-design, Dana is recognized for her unwavering focus on meeting the ever-changing and unique needs of families.

 
Daniel McGoey

Daniel McGoey, Executive Director of Wesway, recognizes the importance of community engagement and innovation in service delivery. This belief has helped guide his organization to meet with success as it has expanded to both address respite care for new populations and to broaden delivery to include communities within several hundred kilometres east and west of Thunder Bay. As a father of four children and soon to be grandfather to seven grandchildren, Daniel is committed to ensuring family-focused care is positioned for future generations. Daniel challenges us to recognize changing demographics and societal trends as we share a joint responsibility as stakeholders committed to creating a responsive service system that places families first.

 
Janet Legge McMullan

Janet Legge McMullan is a Clinical Program Lead with the OACCAC and is the Client Service Lead for the CCAC Assessment Solution Project and the Task Group work that has collaborated with the University of Waterloo to develop new evidenced informed clinical decision support algorithms. She has a broad experience working in health care administration, professional practice, operations, and project management. Janet attained both her Bachelor in Science Nursing and Master in Nursing from the University of Toronto.

 
Bozena Michalik

Bozena Michalik has worked in the community support sector for 19 years. In addition to leading the Quality Improvement Program at ESS Support Services, Bozena is involved in strategic planning, program and fund development, and risk management initiatives. She is also an instructor in the Gerontology Graduate Certificate Program at Humber College. She holds a MA from the University of Guelph, and has completed the Advanced Health System Leadership Program, University of Toronto.

 
Linda Mollenhauer

Linda Mollenhauer has created a non-profit venture called Ignite NPS to develop and support practical research and resources for the leadership of non-profit organizations across the sector. It’s providing her an opportunity to give back to the sector. She was a consultant for over 20 years and, working collaboratively with funders, non-profit organizations and consultants, developed resources such as Collaboration Coach, Benchmarks of Excellence for the Nonprofit Sector; Building Nonprofit Networks; A Culture of Accountability; and Building an Inclusive Organization. She was also CEO of IMAGINE Canada and helped create its national social marketing campaign on giving and volunteering.

With over 35 years in the healthcare sector, Karen has worked extensively and passionately in QI in tertiary centres such as London Health Sciences Centre, and in Primary Care with organizations such as the Quality Improvement and Innovation Partnership (QIIP), Health Quality Ontario (HQO), and Ontario College of Family Physicians (OCFP).

Ian Parker is a co-founder of Ontario’s innovative Self-Managed Attendant Services – Direct Funding Program, administered by Centre for Independent Living in Toronto (CILT). He was the program’s Manager for many years and is now Senior Advisor. Ian has lived with a spinal cord injury and used attendant services for 40 years. He has negotiated with government on policy development and continues to promote the needs of attendant service consumers. In his personal life, he is married and is a proud dad.

Her primary focus is using documentary storytelling techniques to communicate and integrate user needs into decision-making and planning cycles.

Dr. Mike Saks has served on the Executives/Boards of six universities in the UK, including as Provost/Chief Executive. He currently holds Visiting Chairs in Health Policy at the University of Toronto and the University of Lincoln. He has received several international academic awards and published widely on professions and the healthcare workforce, including over a dozen books. He has been a chair/member of many National Health Service committees in the UK and a long-term adviser to governments and professional bodies on the regulation of health and social care – last year receiving the Best Brains Exchange Award from Health Canada and the Canadian Institutes of Health Research for his contribution to federal government and the provinces and territories on scopes of practice.

Michael Sawyer, Director of Heath System Transformation at the Champlain LHIN, is currently responsible for community transformation projects primarily focusing on integrative care models between the Community Care Access Center and Community Support Services. As well, he is a provincial co-chair of the Early Adopter Working Group that is overseeing the implementation of the recent amendments to the Home Care and Community Services Act. Mike has been in health care for most of his career working as a clinical pharmacist and as a consultant in the area of electronic health records.

Pamela recently earned first prize as part of PR Daily’s Video Awards in the category of “Best Event Video” for VHA’s creation and promotion of its Heart of Home Care Award videos, which profile the challenging and inspiring lives of Ontario caregivers.
