CHWs and Partners Take Action to Reduce Heart Health Disparities:
Lessons from NHLBI's Strategic Champions Demonstration Projects

We can’t eliminate health disparities alone, so how can we leverage our resources/work and be part of the solution? Join us to hear about strategies and outcomes from NHLBI's Strategic Champion Demonstration Projects.   Each project used NHLBI's community health worker (CHW) heart health curricula to forge partnerships and make meaningful change in their communities. Hear how Champions trained CHWs to help change attitudes, improve knowledge, and increase heart  healthy behaviors to prevent and control heart disease in racial and ethnic minority communities across the country. This webinar will also engage partners in a conversation on how NHLBI's materials can be used in different communities to help reduce heart health disparities. You will hear about our overall work with the Champions, and three sites will share their unique experience serving specific populations- Hispanic/Latino, African American, Filipino, and Native American.
