Activity Objectives
- Identify patients appropriate for home modality and develop strategies to mitigate any barriers identified (All attendees).
- Recognize catheter malfunction issues and make appropriate and timely referrals (PD Nurses, Renal Case Managers, Surgeons, Nephrologists, and Interventional Radiologists).
- Describe the clinical benefits of peritoneal dialysis and home hemodialysis to patients resulting in increased utilization of the therapies (Nephrologists and Renal Case Managers).
