Welcome to the Kaiser Permanente registration site for Chronic Disease Self-Management Program (CDSMP) and Tomando Control de Su Salud (Tomando) leader trainings.

We are pleased that you are registering for a leader training.  There is no registration fee, however we ask for these commitments:

• Attend all days of the 4-day training
• Arrive on time and be prepared for interaction and public speaking
• Plan to lead a Self-Management workshop within 6 months of your leader training
• Provide licensing information from the organization where you will be leading workshops
• Become familiar with the Stanford guidelines for self-management programs

 It is recommended that you have participated in a CDSMP or Tomando  workshop before becoming a leader.

This is a four day training. Dates for this event are February 15, 16, 22, 23, 2017. Participants are required to attend all four days.
