Close of Early Bird Registration Sunday 17 August 2014.
We encourage you to register online via the button on the right. You do not have to pay immediately by credit card, and during the online registration process can elect to pay by direct credit or cheque at a later date. The online system will email you an invoice for presentation to your funding body who can then arrange payment by either of the three payment methods on your behalf.
On submission of your registration a confirmation will be emailed to you, which includes a link to your registration summary and invoice/receipt. To amend any registration details, or to cancel your registration, please contact the Registration Manager via email jo@outshine.co.nz or phone +64 7 823 1910.
All prices are in New Zealand Dollars, and include GST of 15%.

Payment
Payment can be made by cheque (payable to Breast Care Nurses Education Service), direct credit, or credit card (Visa or Mastercard). Credit card statements will show debit as Outshine Ltd. All prices are quoted in New Zealand dollars and include GST. Cheque payments can be sent by post to Outshine Ltd, PO Box 896, Cambridge 3450, New Zealand.

Download and complete the registration form below and keep a copy for your records. Your registration fee must accompany the completed registration form. Completed registration forms and fees should be sent to the Registration Managers. Confirmation and a tax invoice will be sent via email upon receipt of your form and fees.

Terms and Conditions/Cancellations
All cancellations must be made in writing to the Registration Manager jo@outshine.co.nz. Cancellations made prior to 17 August 2014 will be refunded in full, less a $20 administration fee. Registration refunds will not be made after 17 August 2014, though replacement delegates are welcome. If, for reasons beyond the control of the Organising Committee, the conference is cancelled, the registration fees will be refunded after deduction of expenses already incurred.
Privacy
Personal information is collected on behalf of Breast Health 2014. A delegate list will be produced for the conference including name, organization and email address. It will be distributed to meeting attendees, sponsors and exhibitors and provided to the organisers of Breast Health meetings. This list will not be provided by the Organising Committee to any other third party. Should you not wish your details to be included on the list please indicate when asked during the registration process.
