Please join us for some hands-on practice using the tools in crmViewÔ to manage customer records, build list criteria, and create campaigns. Learn about the latest enhancements and bring your questions and ideas for campaigns you'd like to create. You will learn how to locate, edit, and add records for customers and prospects. You'll set the properties, define the geography, and establish exclusions and inclusions for sample lists. After linking list criteria to a campaign, you'll set up the campaign and retrieve exported mail lists. At the conclusion of the session, a product update will be given by Product Management, including review of upcoming enhancements and time for Q & A.

Presbyterian is piloting the use of Thomson Reuters Likelihood of Hospitalization scoring as a component of the evolution of our existing Patient Centered Medical Home programs to expand focus on reducing hospitalizations. Two of the larger PCP groups are participating. One is part of our integrated health system in the urban area of Albuquerque and the other is a well organized group of rural-based Federally Qualified Health Clinics. One of the key factors in developing this pilot is to not only provide data that are actionable, but also for the health plan to provide training and tools for processes and skills needed to implement and maintain effective outreach to patients at risk. This presentation will cover the development of the program including the payment model, creation of outreach lists, the use of the patient health record, development of desktop tools and tracking systems, considerations about the overlap/coordination with the health plan Complex Case management programs, and building the methodology for measurement of outcomes.
