Keynote Presentation INTERACT II Interventions to Reduce Acute Care Transfers from Long Term Care: The INTERACT Quality Improvement Program
Join Dr. Joseph Ouslander, Associate Dean for Geriatric Programs, Charles E. Schmidt, College of Medicine, Florida Atlantic University for a discussion of a comprehensive quality improvement program to manage acute changes in resident condition and improve every day practice in long term care homes. Dr. Lori Schindel Martin, Associate Director, SRC, Daphne Cockwell, School of Nursing, Ryerson University, will discuss implications of the INTERACT II program for long term care in Ontario.
10:30 AM CONCURRENT SESSIONS:
AM1: Clinical Leadership in LTC
This session will review the increasingly specialized roles of LTC physicians and nurses, new education and training initiatives for medical directors and directors of care, and evidence-based approaches for improving nurse-physician collaboration.

AM2: Scope of Practice
This session will discuss NP/RN/RPN/PSW scope of practice, including how LTC can promote greater role clarity and move forward on the recommendations of the Health Professions Regulatory Advisory Council (HPRAC).

AM3: Person-Centred Dementia
This session will highlight tools for implementing person-centered dementia care and best practices for managing responsive behaviours based on research by the Alzheimer Society of Canada, Partnerships in Dementia Care Alliance, Shepherd Village and Bloomington Cove.

AM4: Rehabilitation & Restorative Care
This session will review the evidence on rehabilitation of the frail elderly and discuss challenges and opportunities for long term care to improve quality of life and outcomes for residents in both short stay and long stay programs.

AM5: Knowledge Exchange
This session will review barriers and opportunities related to the uptake of evidence in long term care, provide practical tips for engaging LTC homes and staff and discuss how to create successful research and poster/presentation submissio

1:15 PM CONCURRENT SESSIONS:
PM1: Data Quality
This session will review LTC data holdings, present research on the quality of those data sets and discuss evidence-based approaches to ensuring high-quality data, including practical tools homes can use to validate their data and promote a data quality culture.
|
PM2: Improving Transitions
This session will discuss the impact of the Home First Program in reforming the delivery of seniors care in Ontario and provide a profile of ALC patients and others waiting for long term care. It will also review transitions at end of life for those living in long term care.

PM3: Quality Matters
This session will present early results from the Determinants of Quality in LTC Study, including impact of the Long Term Care Homes Act and Residents First. It will also profile homes that have achieved significant improvements in performance on RAI MDS and other indicators of quality.

PM4: Pay for Performance
This session will sift through the evidence on Pay for Performance and Quality-Based Funding in long term care. Early results from the Alberta LTC Quality Incentive Program and implications for Ontario will also be discussed.

PM5: ORC Emerging Researchers in Aging Care
This session will showcase the work of early researchers and award recipients of the Ontario Research Coalition of Research Institutes/Centres on Health and Aging, including research on palliative care, cognitive and physical functioning, falls prevention and more.

2:55 PM CONCURRENT SESSIONS:
PM6: Resident & Family Experience Surveys
This session will review tools commonly used to assess family and resident satisfaction in long term care and present the latest findings based on the interRAI QoL Survey, NRC Picker/ myinnerview, NHCAHPS and the Quality Inspection System (QIS).

PM7: Medication Safety
This session will review benzo prescription patterns in Ontario LTC homes and practical approaches to reducing the inappropriate prescribing in long term care.

PM8: Chronic Disease Management
This session will review barriers to heart failure management in long term care, use of bisphosphonate and vitamin D to manage osteoporosis and prevent high risk fractures, and patient order sets to support evidence-based care.
|
PM9: Palliative Care
This session will discuss long term care homes as places to receive quality end of life care. Ontario research on palliative/end of life care will be complimented by presentations from homes working to improve residents and families.

PM10: ORC Emerging Researchers (cont’d)
The ORC builds research capacity with incentives to encourage people early in their research careers to focus on health and aging. Featured presentations (and posters) will discuss the aging brain and care and service delivery across the continuum.
