Tuesday August 5, 2014 – Day 1

9:00 am - 10:00 am Coronary diagnostic and intervention procedures
In the last five years, both caths and intervention coding has been completely revamped. Review the coding principles and changes in both diagnostic cath coding and all the new PCI rules. Apply these concepts to actual case coding to make sure that all the concepts are really understood.
10:00 am– 10:30 am Invasive and Interventional Peripheral Vascular Procedures Part I
First it was bundled carotid stents and leg interventions. Since then we have had completely new codes and principles for coding renal angiography, carotid/vertebral angiography and most recently new vascular stent codes. We will review the various concepts and rules that apply to each body area for both invasive diagnostic and interventional PV coding and then apply these rules to actual case notes.

1:45 pm – 2:45 pm EP Studies, Ablations
In 2013, our diagnostic EP study codes were bundled with new Ablation codes. After several coding updates, we are now able to determine what is separately billable and what is not in the setting of EP Study with Ablation. Learn all the rules and then apply the concepts in our case coding session.
2:45 – 3:10 pm Break
3:10 pm – 4:00 pm Device implants
Pacemaker and ICD implants can be challenging to code. From new system implants to generator changes and upgrades – this modality has a language all its own. We will review all the procedures, concepts and codes to gain a clear understanding. To make sure we can apply the concepts we will code some practice notes to assure our understanding.

Wednesday, August 6, 2014 – Day 2

9:00 am – 10:30 am Non-invasive diagnostics (Echo, Nuclear Medicine, Holters, Event Records, Device Monitoring etc.)
This session provides an overview of the tools and technology used for Cardiology Diagnostic testing. How are these services coded? What are some of the payer guidelines for reimbursement?
10:30 am – 10:50 am Break
10:50 – Noon Diagnosis Coding
Are you coding to the highest level of specificity contained in your documentation? Or are you coding higher? More specific than your documentation supports? Focus on application of ICD-9 and ICD-10 coding.

1:00 pm – 2:15 pm Evaluation and Management (E/M) Part I
  • What are the rules? Review of the 1995 and 1997 guidelines 
  • E/M coding guidance – What are the gray areas? How do individual payer guidelines differ? (eg. Exams, Medical Decision Making, NPP Guidelines, Subsequent Hospital Visits, etc.) 
  • Apply the E/M rules and audit Cardiology Specific notes.

  • Clinical Documentation Improvement 
  • Common EMR/EHR pitfalls – What is cloning? Effective use of scribes 
  • Signature requirements 
  • What is happening with HIPAA audits? 
  • E/M case coding – What are the areas of focus?
3:30 pm – 4:00 pm Coding Exercises 
Review additional E/M notes, see what’s going with actual notes that have been audited.
