Welcome to the on-line registration process to confirm your attendance at the Adaptive Phage Therapeutics Investigator Meeting. The meeting will be held in our Gaithersburg office and we have blocked hotel rooms for the night prior to the event.
A variety of available dates, listed below. Please select the date that best suits your schedule. Please, do not contact the hotel directly. We will be sending a rooming list directly to the hotel based on your registration. A block of 10 rooms is available per date. First come, first served.

Attendance of the Principal Investigator and the Study Coordinator is required. Each site should send a total of two (2) representatives, one from each of these roles. Deviations from these requirements must be pre-approved by APT - Clinical Operations.
Flights
You will be responsible to book your own economy class flights. The meeting will be from 9:00 am to 3:00 pm on the meeting dates. We are holding accommodations for you at the DoubleTree Gaithersburg the night prior to the meeting. Please book your flights accordingly. The meeting ends at 3:00 pm, we recommend that you do not book return flights prior to 6:00 pm in order to ensure ample time for your return airport transfer.

Expense Reimbursement
• Round trip coach/economy travel from your home city (you will be responsible for booking your own air)

Original receipts will be required and an expense form will be provided by email to all participants on-site. Incidental charges such as movies and in room dining, bar charges will not be reimbursed.
Meals
Breakfast and Lunch will be provided by Adaptive Phage Therapeutics on your selected meeting date.
Incidental Account
The Doubletree Hilton will keep a separate account for your personal expenses such as valet and laundry service, personal bar bills, telephone calls, room service charges, and hotel shop purchases. You will need to settle this incidental account upon checking out of the hotel. The hotel accepts all major credit cards.
