CONTINUING EDUCATION

Fulfill your potential with continuing education (CE) hours. While ongoing learning is essential to ensure the best patient care outcomes, it’s also an opportunity for you to explore your own interests or even become a recognized expert in your field. Earn CE hours by attending an ACMA Chapter Conference to meet your license or certification renewal needs and advance your career.

After attending this conference, you will be able to:

  1. Describe longitudinal care regulatory and payment changes and the impact on patients, operations and expectations when working with community partners.
  2. Explore the potential long-term care financial burden for patients and payment mechanisms to access needed services.
  3. Use case examples to examine common ethical dilemmas and the relationship of values in the practice of caring for patients.
  4. Analyze strategies to support health care teams and promote resilience while meeting evolving patient and population health needs.

5.0 Nursing and Social Work Contact Hours (based on 50 or 60-minute hour)
5.0 ACM & CMAC

ACM & CMAC – This CE activity is approved by the American Case Management Association (ACMA) for ACM & CMAC recertification credits.

Through a collaborative agreement with the Commission for Case Manager Certification (CCMC), this activity and the corresponding ACM/CMAC credits are acceptable for CCM renewal.

NURSING CONTINUING EDUCATION

The maximum number of hours awarded for this live Continuing Nurse Education is 4 contact hours.
The maximum number of hours awarded for this enduring Continuing Nurse Educaiton is 1 contact hour.

 

SOCIAL WORK CONTINUING EDUCATION

Please Note: This approval applies to Session 1, 3, 4 & 5. These conference sessions, courses #3762-3766, are approved by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program to be offered by American Case Management Association as individual conference sessions. Individual courses, not providers, are approved at the course level. State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit. ACE approval period: 08/23/2021 to 08/23/2023. Social workers completing this conference receive UP TO 4 continuing education credits.

Outpatient Dialysis Coordination: Challenges & Solutions, Course #3352, is approved by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program to be offered by American Case Management Association as an individual course. Individual courses, not providers, are approved at the course level. State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit. ACE course approval period: 12/01/2020 - 12/01/2022. Social workers completing this course receive 1 General Social Work Practice continuing education credits.

METHOD OF PARTICIPATION AND REQUEST FOR CREDIT

Attendee Completion Requirements:

In-Person (live-stream and live in-person sessions)
Must sign session sign-in/out sheet upon entering and leaving session.
Must attend full session.
Must complete/submit an electronic evaluation.
No partial credit will be given.

Distance Learning/Enduring Sessions
Must log-in/out on time. You must complete a post-test prior to evaluating the session. Per continuing education regulations, you MUST complete the full length of each session in order to qualify for the CE credit. Within the session there is a Finish button, clicking this button is a REQUIRED step.   

Request for Credit
You will receive an email from ACMA 1-3 days post-conference notifying you that the electronic posttest/evaluation is available for the conference sessions. Detailed login instructions will be listed in the email. Once you log in and complete, you will have immediate access to download and/or print your CE Certificate.

 

As required by the boards and approval agencies listed above, ACMA maintains records and CE certificates for all educational activities for a minimum of seven years after the educational activity concludes.
