  Learning Objectives  
         
  1. Identify and implement next steps to increase PD rates.  
2. Obesity – follow the SCPMG position for adult weight management and provide evidence based, consistent education and care to the patient.
3. Manage immunosuppression in Kidney Transplant patients utilizing the current evidence based medicine.
4. Examine methodology, current systems, and evidence based literature, to utilize care that is clinically proven, necessary and integral to improving patient care and safety.
5. Nephrologists use shared decision making to determine Dialysis options and Life Care Planning.
6. Build a community for learning, collaboration, development and implementation of best practices with the ESRD interdisciplinary care team to improve outcomes care management.
