Cancellation Policy for Registration: Full refunds, less a $50 processing fee, will be granted if written requests are received by AGD on or before 5 p.m. CDT June 23, 2017. Cancellation requests received after the June 23 deadline will not be eligible for credit or refund. Please email your request to meetings@agd.org or fax to 312.440.0513.
Registration and attendance at, or participation in AGD 2017 and other associated activities constitutes an agreement by the registrant to permit the Academy of General Dentistry to the use and the distribution of the registrant or attendee's image and/or voice in photographs, videos, electronic reproductions, audiotapes, and in any media, now or hereafter contemplated, in perpetuity, in the sole discretion of the AGD.
