20th Anniversary Meeting of the European Association of Osseointegration
 

Registrations for the 20th Anniversary Meeting of the European Association for Osseointegration will be made online via the conference website. No registrations will be accepted by email, post or phone.
Registration fees are based on the date of receipt of the registration. The prices indicated are per person and are only applicable if both the registration form and the payment are received before the deadlines above, otherwise the higher registration fee will be applied. 
Late registrations will close on October 1 2012 at 18.00 (GMT + 1.00). Onsite registrations will be available on October 10 2012.

The conference registration fee includes: 
- admission to the congress sessions, poster areas and technical exhibition
- congress documents (final programme, abstracts books, congress bag)
- lunches and coffee breaks
- the welcome cocktail on Wednesday, October 10. 
- local VAT (25 % on registration fee). 

Member registration 

Please note that only direct members of EAO, having paid their 2012 membership fees, and members of  the Scandinavian Society of Periodontology, the Scandinavian Society for Prosthetic Dentistry, the Scandinavian Association of Oral and Maxillofacial Surgeons and the Danish Society for Oral Implantology can benefit from the member registration fee.
If you are not listed on these associations' membership lists, the organisers reserve the right to register you as a non-member and charge the corresponding registration fee. Non-members who wish to pre-register as EAO members may go to the EAO website to submit a membership application before registering. 

Student registration 
Once registered, students should provideby fax (+33 1 44 64 15 16) or email (eao2012@clq-group.com) a valid student identification confirming their undergraduate student status before October 1, 2012, otherwise the higher registration fee will apply onsite. 

Group registrations

A special rate is available for group registrations according to the following conditions:
• Minimum of 5 participants
• Deadline for submitting a purchase order : August, 31 2012
• Invoice settlement upon receipt
• Deadline for payment : September, 16 2012
• Unique rate for all registration types (590€ VAT included) : no reduced fees will be applied
• Your registration interface will be available until September, 30 2012

Children

To provide all EAO attendees an optimal environment, children under the age of 18 are not permitted into the scientific sessions or the exhibition hall during show hours, installation or dismantling. 
Childcare is not available at the conference and exhibition center; however your hotel concierge may be able to assist you.

Payment 
Payment must accompany the registration form. Registrations received without payment will not be processed nor confirmed. 

Your credit card transaction will be processed through Colloquium Paris (EAO Organizing Secretariat). Please note that your monthly credit card statement will indicate payment to "12EAO - CLQ".

Confirmation 
Receipt of your registration form and credit card payment will directly be acknowledged by email. If you have not received a confirmation, please contact eao2012@clq-group.com or call +33 1 44 64 15 15.

VAT 
Danish VAT is applicable on the registration fees (25%). Please note that VAT on registration fees paid by companies and other taxable entities can only be reclaimed if the customer’s VAT number is mentioned on the invoice. If applicable, please fill out your company’s VAT number in the section “VAT number” on the registration form. 

Invoice 
Only digital version of invoices will be sent to participants. The invoice will be issued within one week after the registration and cannot be changed, so please make sure you have provided us with the correct information (company name, address and VAT number if applicable). Cancellation policy 
Cancellation policy for individual registrations:

• Before July, 16th: Reimbursement of 50% of the fees paid. With no payment received before cancellation, this amount will still remain due. 
• After July, 16th: No refund. With no payment received before that date, the total amount will still remain due to the Congress office.
All refunds will be issued after the congress. Requests will not be accepted after the 15th of November 2012.

Name change 
Registered participant who are unable to attend the congress will not have the possibility to nominate a substitute participant. Name changes will constitute a new registration.
