Please note you must have your badge scanned at the event and complete the feedback form to receive a CPD certificate for CHOLE 2016.
Once your feedback form has been received, a certificate of attendance will be sent to you via email. The deadline for completing the evaluation form in order to receive a certificate is Friday July 15th 2016.
Please note, we will not be issuing any certificates after this date.
